  • 8:00am Maureen’s asleep lying on her Lt side. As per Marie, she was awake earlier at 7:30 when clocking in but fell asleep soon after.

  • 8:30am Maureen’s asleep.

  • 9:00am Maureen is still sleeping.

  • 9:30am Maureen opened her eyes; we repositioned on her right side and with a swab wetted her lips.

  • Carolyn visited Maureen:)

  • 10:0am We raised Maureen’s bed head and administered Levothyroxine with apple sauce. Maureen is very sleepy today, she went back to sleep after taking her medication.

  • 10:30am Marie and I performed a full body bath, changed and cleaned a md wet diaper, patted clean Maureen’s sacral pressure spot with a damp towel, dried and applied Cavilon barrier and cream. Sacrum pressure spot seems to be healing well. Dressed and headed to the kitchen for breakfast.

  • 11:00am Maureen is enjoying an oatmeal/fruit smoothie and 8oz prune juice with 1/4cap Miralax for breakfast. Coughed twice during breakfast.

  • 11:25 Administered Lisinopril and loratadine with smoothie. Reclined Geri chair one notch.

  • 11:30am Oral care and facial routine.

Daily Miralax dosage: 1/4cap with 8oz prune juice.

  • 12m Maureen is snoozing in the chair. Reclined Geri chair all the way.

  • 1:00pm Maureen sleep in the chair for over one hour. She seems tired and opened her eyes only after we encouraged by talking to her.

  • 1:05pm Lunch: potato soup, tuna salad and steamed cauliflower. Maureen’s eating without much interest- omitted yogurt this time. She had half of her first cup of lemon water, no much chance to drink water when she’s sleeping.

  • 1:35pm Oral care.

  • 1:45pm Headed Maureen to the bedroom for today’s only rest break. Changed and cleaned a md wet diaper and positioned on her right side for an extended break at 2:00pm. Pulse 68, O2 94.

  • 3:30pm Today’s break ends. Diaper was dry, dressed and washed Maureen’s hair. Towel dried and styled with moose. Headed Maureen to the front room at 4:00pm.

Maureen’s Sacrum area Assessment

A red spot on the sacrum area was observed last Saturday during Maureen first change. It was confirmed to be pressure spot when it was more visible by her second diaper change. Sacrum area, besides been treated with the usual cavilon barrier and cream was also let opened during rest breaks since Thursday as suggested by the nurse during the social worker visit last Wednesday. Pressure spot seems to be healing well, a reduction in size is noticeably today.
