  • 9:00am Maureen is sound sleep. She is lying on her left side. She has been asleep the whole time.

  • 9:05am Maureen is awake.

  • 9:10am Merianne and I helped Maureen to a sitting position in bed. Administered 75mcg Levothyroxine with applesauce.

  • 9:20am We begin our morning with a partial bed bath using warmed wipes from the purple packet. Maureen's diaper is dry - Merianne and I performed a clean and change. No discharge present. Applied Cavilon barrier cream to appropriate areas and Cavilon barrier film to the sacral area. Also applied Calazinc to the redness on the sacral area. We dressed and headed to the front room via Hoyer lift.

    • Here is a picture of the redness. Merianne and I are in agreeance that it is a Stage 1 pressure ulcer.

  • 9:35am 10 oz lemon water.

  • 9:45am Breakfast -onion, tomato and cheese omelet with pear. 8 oz apple juice/water.

  • Daily miraLAX dosage - none

  • 10:03am 10mg Loratidine with applesauce.

  • 10:08am Oral care.

  • 10:15am Nebulizer treatment.

  • 10:25am Morning facial care routine.

  • Vitals: BP 106/62, temp 97.9, pulse 58, O2 97%.

  • Maureen is fully awake for most of the morning with eyes wide open. Lung sounds are clear, vitals stable. Coughing is minimal this morning. The stage 1 pressure ulcer on the sacral area is healing. Maureen fell asleep on the recliner in the front room from 10:50am until 11:30am when we headed to the bedroom for the first rest break.

  • 11:30am We headed to the bedroom for rest break #1. Maureen's diaper is medium wet - Merianne and I performed a clean and change. Maureen, with two green wedge pillows secured firmly behind her, will lie on her right side for one hour starting at 11:38am.

  • Merianne reported to me that she has seen low oxygenation levels in Maureen. I elected to keep the pulse ox on Maureen's left index finger for 30 minutes. Her average oxygenation levels in this time frame, at rest, is 90% - 92%. Respiratory rate is 18.

  • 12:38pm Rest break #1 ends. Maureen's diaper is dry. We dressed and headed to the front room via Hoyer lift.

  • 12:46pm Lunch - shrimp salad, sliced tomato, 10 oz root beer. Rice pudding.
