July 6, Monday, Day Shift

TOM:

7:15 - Maureen was coughing a couple times, I gave her some water, via swab, went back to sleep.


Vitals at this time …


Temperature- 97.0°

Pulse- 76BPM

Oxygen- 94%


Back to sleep …


8:40 - two coughs, one suction, four more swabs of water …

MUCH MORE ALERT THAN YESTERDAY!


9:30 am:

Maureen is sleeping.

9:40 am: Maureen wakes up.

We gave her few sips of water via swab.

We placed Maureen in an upright position.

9:50 am: Administered 75mcg of Levothyroxine with apple sauce.

2 oz of water (with thickener).


10:00 am: Shelly - the nurse visited Maureen.

Carolyn and Colleen came.


We went through the chain of events from Saturday.

Temperature - 98.8°

Pulse - 88BPM

Respirations of 18


Lungs are bowel sounds are good.


Breast has no change, that’s a good thing


If LORAZEPAM is needed again, start with .25ml


11:00 am: Maureen felt a sleep again.

She is still in bed.


11:45 am: Me and Martha washed Maureen’s upper body with warm purple wipes.

Dry diaper. Medium brown discharge.

Cleaned and changed.

Applied Sureprep barrier film on sacrum and Prevent Silicone Cream on inner thighs.


We applied Triamcinolone Cream on her right foot.

We washed her left breast with gauze sponges and Skintegrity spray.


Coughing- 3 times

Section- NO


12:15 am: We transferred Maureen to the living room.


12:20 am: Breakfast: Scramble egg with cheese and guacamole.

5 oz of water with lemon and thickener.

Administered 10 mg of Cetirizine Hcl with apple souce.

Administered Geri-Tussin.


Coughing- 2 times.

Section - 2 time.

4 oz of prune juice with thickener.


1:15 pm: Nebulizer.


1:30 pm: Oral care.


1:35 - 1:45 pm: Nap


2:10 pm: We transferred Maureen to the bedroom for break-rest.

Light to medium diaper.

We cleaned and changed.

Positioned on her right side.


3:10 pm: Maureen began to moan softly, lifting her legs and clutching her abdomen.

We gave her 500 mg of acetaminophen mixed with applesauce and a thickener.

We fed it to her in tiny amounts using a teaspoon. After swallowing the entire dose, she started to cough. We positioned her on her left side. One section.

3:30 pm: She seems very comfortable and at ease. The medication has started to take effect.

We will wait few more minutes and we will transfer her to living room.

3:50 pm: We transfered her to the living room.

4:10 pm: 10 oz of sprinkle water.

Full small bowl of soup.


4:45 pm: Maureen didn't take any medication in the evening—just a little bit of OLANZAPINE (25%) with yogurt and thickener.

We tried giving her Vimpat mixed with blended watermelon, but she kept spitting it out.

Our attempts to administer her medication lasted 45 minutes—with no success.

She was tired.


6:00 pm: We took Maureen to the bedroom.

Dry diaper. Medium brown discharge.

Cleaned and changed. Administered Prevent Silicone Cream.

We applied Triamcinolone Cream on her foot.

We washed left breast with gauze sponges and Skintegrity.

We applied Cetaphil lotion to face, hands and feet.

Positioned Maureen on her left side.

P-96

6:30 pm: Maureen is in bed.
