9:30 am: Maureen is awake.


9:35 am: We placed Maureen in an upright position.

2 oz of water with lemon and thickener

Administered 75mcg of Levothyroxine with apple sauce.

4 oz of water with lemon and thickener


10:15 pm: Maureen fell asleep again for an hour.


11:15 am: Me and Tom washed Maureen’s upper body with warm purple wipes.


Dry diaper. Medium brown discharge.

Cleaned and changed.

Applied Sureprep barrier film on sacrum and Prevent Silicone Cream on inner thighs.


We applied Triamcinolone Cream on her right foot.

We washed her left breast with gauze sponges and Skintegrity spray.


Coughing- 2 times

Section- 1 time


Blood Pressure 110/70

11:50 am: We transferred Maureen to the living room.


12:05 am: Branch: scrambled egg with tomato pepper and cheese. Avocado. Banana.

Administered 10 mg of Cetirizine Hcl.

12 oz of orange juice with thickener.


Coughing- 6 times.

Section - 4 times


12:50 pm: Nebulizer.


1:05 pm: Oral care.

Face routine.


1:45 pm: yogurt

10 oz of sprinkle water with thickener.


Coughing - NO

Suction - NO


2:45 pm - we took Maureen to the bedroom for break-rest

Dry diaper.


3:45 pm: We transferred Maureen to the living room.

Dry diaper.


4:30 pm: Dinner: lemon chicken soup with thickener

8 oz of water with lemon and thickener.


5:00 pm: Administered 5 mg of Olanzapine with apple sauce.

5:10 pm: Administred: 12,5 ml of Vimpat


Coughing- 2 times

Section- 2 times


5:30 pm: Oral Care.


6:00 pm: Transferred Maureen to the bedroom.

Dry diaper.


When we laid Maureen on the bed, we decided we should give her Tylenol. She was tired, but the pain was visible on her face.

After the medication was administered, she began to cough violently, could not clear the mucus from her throat, and could not catch her breath.

We used suction machine, but it didn’t help.

After Tom called JourneyCare, we administered Lorazepam 1 ml and Morphine 0.25 as recommended by the nurse.

Maureen began to relax; she was gradually calming down. Maureen, in the midst of coughing did: Large formed BM and the diaper was very heavy wet.


We Cleaned and changed. Applied Prevent Silicone Cream.

We applied Triamcinolone Cream on her foot.

We applied Cetaphil lotion to face, hands and feet.

Positioned Maureen on her left side.

7:35 pm: she felt a sleep.
