• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:45pm Checked diaper, it’s dry.

• 4:50pm Dinner: Fried chicken with French fries and half cup of lemon water. Coughed three times.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper and Small-soft BM, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: Temp: 97, Pulse: 68, O2: 97.
