• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:45pm Diaper it’s light wet.

• 4:50pm Dinner: Chicken and broccoli casserole with half cup diluted cranberry juice. Maureen coughed three times.

• 5:25pm Administered 1-50mg Senna plus (crushed) with apple sauce.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper with slight trace of vaginal discharge, applied Cavilon barrier cream, applied nystatin to vaginal redness. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: temp: 97, pulse: 67, O2: 96.
