• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 101/70.

  O2: 97

• 4:45pm Albuterol Neb treatment.

• 4:50pm Diaper is light wet.

• 5:00pm Dinner: Tuna bake and 5oz lemon water. Maureen coughed twice during meal.

• 6:00pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  6:10pm Oral care.

  6:20pm Transferred Maureen to the bedroom. Tom and I clean/change a heavy wet diaper with a large-formed BM, applied Cavilon barrier cream. Clean/applied calazinc to both, abdominal and left armpit redness. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

  Positioned on her left side, Maureen goes to sleep for the night.
