• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 116/69.

  Pulse: 81

  O2: 98

• 4:50pm Diaper is light wet.

• 4:55pm Dinner: Fish and chips with avocado mashed, and 7oz of root beer. Maureen coughed four times during meal.

• .5:55pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  6:05pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Tom and I clean/change a heavy wet diaper, applied Medline Remedy cream.  Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

  Temp: 96.5

   Pulse: 76

   O2: 98

  Positioned on her left side, Maureen goes to sleep for the night.
