• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:25pm Blood pressure: 100/62, P 64.

• 4:45pm Diaper it’s light wet.

• 4:50pm Dinner: Cod with tomatoe and olives, sweet potato, squash spaghetti, and 1/3 cup of lemon water. Coughed twice.

• Maureen took a little nap after dinner.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with Small-soft BM, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. No vaginal discharge observed.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: temp: 96.7, pulse: 68, O2: 98.
