• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:58 Diaper is light wet.

• 5:00pm Dinner: Chicken tender in cream sauce with cauliflower and spinach, and 6oz lemon water. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6::00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper and a very small-soft BM, applied Cavilon barrier cream, including healing redness by her outer anal area. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Vitals:

  Pulse: 60

  O2: 98

• Positioned on her left side, Maureen goes to sleep for the night.
