• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 108/62, P: 56,

  O2: 99.

• 4:45pm Saline Neb treatment.

• 4:50pm Diaper is dry.

• 5:00pm Dinner: Pasta with cheese, tomato and peas, and 4oz lemon water. Maureen coughed three times during meal.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Tom and I clean/change a heavy wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Vitals:

Temp: 97. 0

Pulse: 73

O2: 96

• Positioned on her left side, Maureen goes to sleep for the night.
