• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 100/60, P68.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Lemon chicken, sweet potato, spinach, and half cup of lemon water. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper with a medium-yellowish vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Positioned on her left side, Maureen goes to sleep for the night.
