  • 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

  • 4:30pm Blood pressure: 100/60, P59.

  • 4:45pm Diaper is light wet.

  • 4:50pm Dinner: Mac and cheese, ham and broccoli casserole, and half cup of lemon water. Coughed twice.

  • 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  • 6:00pm Oral care.

  • 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a medium-soft BM, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. Applied nystatin powder to redness by her right arm pleat, left upper thigh and also to the one on her abdomen (this one looks faded).

    Temp: 96.7

    Pulse: 82

    O2: 98

  • Positioned on her left side, Maureen goes to sleep for the night.
