• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:40pm Blood pressure: 112/61, P 72.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Meatloaf muffing with roasted potatoes and asparagus, and half cup of water with a touch of coconut water. Coughed twice.

• 5:50pm Administerred 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with small-formed BM, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.
