• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:40pm Blood pressure: 116/63.

  Pulse: 83

  O2: 96

• Maureen had 3oz of tea.

• 4:50pm Diaper is light wet.

• 4:55pm Dinner: Baked chicken/potatoes dish with 6oz lemon water. Maureen coughed twice, suction pump used once.

• 5:45pm Saline Neb treatment.

• 5:55pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:10pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Tom and I clean/change a heavy wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

         Temp: 96.7

         Pulse: 71

         O2: 97

• Positioned on her left side, Maureen goes to sleep for the night.
