• 4:00pm Maureen is sitting comfortably in her recliner watching games show.

• Maureen had half cup of ginger tea.

• 4:45pm Checked diaper, it’s light wet.

• 4:50pm Dinner: Jambalaya with corn and asparagus, half cup of water. Maureen coughed once during dinner.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Cleaned and changed a heavy wet diaper, applied Cavilon barrier cream. Wiped clean hands and face, applied Cetaphil cream, including lower extremities. Redness by Maureen’s armpit it’s still visible, applied nystatin powder.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: Temp: 97.2, Pulse: 65, O2: 97.
