• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 115/65, P67.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Park tenderloin, baked potato, spinach, and 4oz lemon water. Coughed once.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a very small-formes BM, applied Cavilon barrier cream, applied Calazinc to left labia fading redness. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Positioned on her left side, Maureen goes to sleep for the night.
