• 4:00pm Maureen is sitting comfortably in her recliner watching games show.

• 4:40 Blood pressure: 100/62, P 66.

• 4:45pm Checked diaper, it’s light wet.

• 4:50pm Dinner: Cod with sauce, mashed potatoes, spinach, carrots, and half cup of lemon water. No coughs.

• 5:50pm Administered 12.5ml Vimpat, 5mg Olanzapine with Apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Cleaned and changed a heavy wet diaper, applied Cavilon barrier cream. Wiped clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: Temp: 97.2, Pulse: 62, O2: 96.
