Sunday, September 10, 2023 Daytime Summary

8:00 AM. Arrived. Maureen is already awake.


8:20 AM. Put her head up and administered 75mcg of Levothyroxine with apple sauce.


9:05 AM. Started morning care. Cleaned her upper body with warm purple wipes and Aloe Vesta and then dried. There was a little redness on her stomach folds, applied Nystatin cream. Diaper was medium wet. Cleaned, dried and changed. Applied Cavilon barrier film on sacral area and Desitin on her anal area. There was no more redness observed. Applied Cavilon cream on the rest of perineal area. Finished dressing up then head to kitchen for breakfast.


9:30 AM. BP was 125/75. Pulse was 54bpm


9:35 AM. Breakfast- omelet, banana, and 2 whole cups of lemon water. 1/4 cap of Miralax was mixed with a cup of lemon water.


10:05 AM. Administered 10mg of Loratadine with yogurt.


10:20 AM. Oral, facial and hand care done.


11:00 AM. Nebulizer treatment done.


General observation: Maureen is more awake and barely nap on her chair.


11:20 AM. Deacon Andy came for Sunday service.


11:30 AM. Transferred Maureen back to bedroom for 1st restbreak. Diaper was dry. Positioner her facing her right side.


12:30 PM. 1st break done. Diaper was heavy wet. Cleaned, dried and changed. No redness noticed on perineal area. Proceed on transferring Maureen to wheelchair. Went to backyard for lunch.


12:50 PM. Lunch was organic fruit smoothie and sparkling water which Maureen enjoyed a lot.


1:30 PM. BP was 100/70. Pulse was 70bpm


2:00 PM. Transferred Maureen back to bedroom for 2nd rest break. Diaper was medium wet. Cleaned dried and changed. Position her to face left side.


2:50 PM. Rest break done. Diaper was medium wet. Cleaned, dried and changed. Proceed on transferring Maureen to the front room.


3:10 PM. Sister Roberta came for Maureen's reiki massage.


3:30 PM. Maureen had her organic fruit snack.


**giving her sips of water in between.


4:45 PM. Massage done.


5:00 PM. Dinner was sweet and sour chicken with rice and vegetables and lemon water. **coughed 1x


5:45 PM. BP was 115/75, Pulse 78bpm


6:00 PM. Administered 5mg of Olanzapine and 12.5ml of Vimpat with apple sauce.


6:25 PM. Oral and denture care done


6:45 PM. Transferred Maureen to bedroom. Her diaper was super heavy wet. Cleaned, dried and changed. Applied Cavilon cream to perineal area. No redness noticed. Applied Cetaphil lotion to upper and lower extremities. Positioned her to face left with 2 wedge pillow and put a pillow in between her legs to avoid any pressure injury.

Pulse: 76bpm

O2: 96%
