Saturday, July 25, 2026 - Daytime Update

9:30AM - Maureen's awake and lying on her R side. Observed moaning and kicking her legs when I arrived however, easily drifts back to sleep when she stops moving and/or feels comfortable.

9:55AM - Currently, her eyes closed and went back to sleep.

**Maureen's in and out of sleep, we gave her water via swab whenever she opens her eyes.

10:30AM - We placed Maureen in an upright position and administered 75mcg of Levothyroxine (crushed) with apple sauce. Administered crushed 1000mg of Tylenol with apple sauce.

BP: 110/65, P: 85, O2: 94%

10:50AM - Me and Tom gave Maureen a full body bed bath using warm purple wipes and Remedy Foam cleanser, no redness or open skin. Cleansed L breast using Skintegrity and clean gauze, no skin discoloration on the area and discharges observed. No redness or swelling observed on R foot.

Diaper was medium wet with small light brown v. discharge. We cleaned and changed her. Applied Remedy Silicone cream on her anterior and posterior inner thighs. Applied Sureprep barrier film on sacrum. We finished dressing Maureen and transferred to the front room.

11:15AM - Neb treatment with albuterol.

11:25AM - Maureen closed her eyes. She had 2-3 spoons of thickened prune juice and a spoon of scrambled egg.

12:30PM - We are still finishing the breakfast/egg with tomato and cheese from this morning. She had 3-5 spoonfuls of thickened sprite.

**Maureen's sleeping in between feeding.

1:00PM - Maureen continues to sleep.

Facial and hand care routine.

2:00PM - Administered 0.25ml Morphine sublingually.

2:20PM - We transferred Maureen to the bedroom for rest break. Diaper was lightly wet with small brownish v. discharge. We cleaned and changed her. Positioned to face the right side.

2:40PM - Maureen showing signs of pain and discomfort by moaning and kicking her legs.

P: 107-109 for 2 mins and went down to 89-93 beats/min after. O2 at 92-94%.

Coughed 2x on bed, suctioned each time.

3:30PM - Maureen had a 30min nap. Rest break is done. Diaper was clean and dry. We transferred Maureen to the front room.

4:00PM - Maureen had sips of sprite and water, had mango popsicle. Coughed 2x, suctioned twice.

Tommy arrived.

4:45PM - Administered 5mg crushed Olanzapine and 12.5ml Vimpat with apple sauce.

5:40PM - Oral care.

6:00PM - We transferred Maureen to the bedroom. Diaper was lightly wet with small grayish v. discharge. We cleaned and changed her. Applied Midline remedy cream on anterior and posterior inner thighs. Cleansed L breast using Skintegrity and clean gauze. Applied Cetaphil lotion on face, arms and legs. Position to face her left side with pillow in between her legs.

6:20PM - Maureen's sleeping.

T - 97.2

P - 98

O2 - 94 %

General Observation:

Observed Maureen to be sleeping throughout the day and would only open her eyes and starts to moan with furrowed eyebrows, kicks and/or move both legs as signs of pain. She did not eat her food today, only took sips of liquid via swab. Appeared comfortable and relaxed after 0.25mg Morphine was administered. Keep Maureen hydrated.
