• 4:00pm Maureen is sitting comfortably in her recliner watching TV.

• 4:30pm Blood pressure: 115/69.

  Pulse: 70

  O2: 96

• 4:45pm Albuterol Neb treatment.

• 4:50pm Diaper is dry.

• 5:00pm Dinner: Grilled hot dog with potato salad and pickle, 6oz lemon juice. Maureen coughed and sneezed five times (dry) during meal.

• 6:00pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  6:10pm Oral care.

  6:20pm Transferred Maureen to the bedroom. Tom and I clean/change a heavy wet diaper with a medium-formed BM, applied Cavilon barrier cream, and also applied a thin layer of nystatin to redness by Maureen’s abdominal fold and by her left armpit. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

   Pulse: 71

   O2: 98

  Positioned on her left side, Maureen goes to sleep for the night.
