4:00 PM. Maureen was sitting comfortably on her recliner chair watching TV.

5:00 PM. Dinner was pasta, fish and spinach and a cup of lemon water. Maureen enjoyed her dinner a lot. Coughed 1x

5:45 PM. BP was 118/70 and pulse was 76bpm

6:00 PM. Administered 5mg of Olanzapine and 12.5ml of Vimpat with apple sauce

6:15 PM. Oral and denture care done

6:30 PM. Transferred Maureen back to bedroom. Diaper was heavy wet. Cleaned, dried and changed. Applied Cavilon cream on her perineal area and Cetaphil lotion on her upper and lower extremities. Positioned her to face left side with 2 wedge pillows. Also put a pillow in between her legs to avoid any pressure injury.
