• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 118/70, P 60.

• 4:45pm Diaper it’s dry.

• 4:50pm Dinner: Manicotti with sausage, spinach, tomato and cheese, and half cup of lemon water. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• Maureen did not show any signs of distress during the evening.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• No vaginal discharge observed.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Virals: temp: 97.1, pulse: 53, O2: 96.
