• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:35pm Blood pressure: 108/62.

• 4:45pm Diaper it’s dry.

• 4:50pm Dinner: Chicken piccata with spinach and asparagus, and 3/4 cup of ginger tea. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper, applied Cavilon barrier cream. No vaginal discharge observed. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: temp: 96.7, pulse:67, O2: 96
