• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 104/61, P67.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Chicken lemone with baked potato and creamed spinach, and 4oz of ginger water. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper, applied Cavilon barrier cream, applied Calazinc to left labia redness. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Vitals:

  Temp: 97.1

  Pulse: 72

  O2: 97

• Positioned on her left side, Maureen goes to sleep for the night.
