• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 105/63, P65.

• 4:45pm Diaper is light wet.

• 4:50pm Dinner: Baked ziti with sausages, and 5oz of lemon water. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a large-formed BM, applied Cavilon barrier cream, including near anal área redness. Noticed some redness up -buttom her left side labia, applied nystatin. Wipe/clean hands and face, applied Cetaphil cream, lower extremities included.
