• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 107/65, P63.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Tilapia roll with tzatziki sauce, potato and carrots, and 7oz lemon water.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  6:00pm Oral care.

  6:20pm Transferred Maureen to the bedroom. Barb and I clean/change a heavy wet diaper with a medium yellowish vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

  Positioned on her left side, Maureen goes to sleep for the night.
