• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 102/60, P71.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Sole roll with sauce, spinach and carrots, 1/3 cup of ginger tea. Coughed once.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with half activia yogurt.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a medium-brownish vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Vitals:

  Temp: 97.1

  Pulse: 70

  O2: 97

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.
