  • 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

  • 4:30pm Blood pressure: 105/64, P74.

  • 4:45pm Diaper is lightly wet.

  • 4:50pm Dinner: Chicken with sun dried tomato sauce, zucchini and tomato, and half cup of lemon water. Coughed three times.

  • 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  • 6:00pm Oral care.

  • 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper, applied Cavilon barrier cream, applied nystatin powder to abdominal redness. No vaginal discharge present. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. Applied nystatin powder to right arm pleat redness.

    O2: 96

  • Positioned on her left side, Maureen goes to sleep for the night.
