• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 105/62, P72.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Fajita casserole with lime chips, and 5oz of lemon water. Coughed three times.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper with a very small-soft BM, applied Cavilon barrier cream, applied nystatin powder to her abdomen left inner thigh redness . Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. Applied a thin layer of hydrocortisone cream to redness spot on Maureen’s left cheek and left arm.

• Positioned on her left side, Maureen goes to sleep for the night.
