• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 103/65.

  O2: 95

4:45pm Diaper is light wet.

4:50pm Dinner: Chicken pasta and 8oz of apple juice. Maureen coughed three times during meal.

4:40pm Saline Neb treatment.

• 6:00pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:10pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Barb and I clean/change a heavy wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

         Temp: 98.4

         Pulse: 67

         O2: 97

• Positioned on her left side, Maureen goes to sleep for the night.
