• 4:00pm Maureen is sitting comfortably in the recliner watching the games show.

• 4:45pm Blood pressure: 102/70, P 57.

• 4:50pm Checked diaper, it’s light wet.

• 5:00pm Dinner: Teriyaki pork loin, potatoes, broccoli and half cup of lemon water. Maureen coughed twice, suction pump used.

• 6:00pm Administered evenings meds: 12.5ml Vimpat, 5mg Olanzapine with apple sauce.

• 6:10pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Cleaned and changed a heavy wet diaper, applied Cavilon barrier cream and nystatin to redness by her abdominal fold. Wiped clean hands and face, applied Cetaphil cream, including lower extremities.

• 6/50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: Temp: 96.7, Pulse: 62, O2: 97.
