TOM - morning 8 am:

Temperature is 97.6°

Pulse is 73 BPM

Oxygen is at 95%

Maureen is awake and taken a few sips of water via swab.


9:30 am: Maureen is awake.

Temp. 97.6


We placed Maureen in an upright position.

We gave her three spoons of water with lemon and thickener.

9:50 am: Administered 75mcg of Levothyroxine with pear sauce.


Maureen looks like she is in pain.

10:15 am: We administered Acetaminophen 500 mg tablet with pear sauce.


6 oz of water with lemon and thickener.


Me and Sheryl washed Maureen’s upper body with warm purple wipes.

Dry diaper. Medium brown and red discharge.

Cleaned and changed.

Applied Sureprep barrier film on sacrum and Prevent Silicone Cream on inner thighs.


We applied Triamcinolone Cream on her right foot.

We washed her left breast with gauze sponges and Skintegrity spray.


BP: 120/72


Pulse is 81 BPM

Oxygen is at 97%


10:40 am: We transferred Maureen to the living room.


She was coughing and sneezing a lot.

We suction a lot of flame.

We decided to do the treatment

11:00 am: Albuterol/ Nebulizer


11:15 am: Administered 10 mg of Cetirizine Hcl and

Geri-Tussin.with pear souce.


11:30 am: Breakfast: 6 oz of prune juice with thickener, scrambled egg with tomato and cheese. Some kiwii.

Face routine.

12.10 - 1:15 am : NAP

Oral care.


Maureen didn’t want to eat yogurt.


2:00 pm: We took Maureen to the bedroom for the break rest.

Dry diaper.


2:15 pm: The nurse from the journeycare visited Maureen.


3:10 pm: We applied suppository BISACODYL 10 mg

We change Maureen’s position to the left side.

3:30 pm: Dry diaper.

We transferred her to the living room.


4:00 pm: 10 oz of coconat water with thickener.

4:15 pm: Nebulizer/ Albuterol

Administered 5 mg of Olanzapine and Vimpat with pear sauce.

Dry diaper

Dinner: lasagna.


A LOT OF COUGHING WITH FLAM AND SNEEZING AND SUCTIONING


5:15 pm : Administered Geri -Tussin with pear and thickener.


5:45 pm: Transferred Maureen to the bedroom.

Very wet diaper. Large formed BM.

Small - bloody vd.

Cleaned and changed. Administered Prevent Silicone Cream.

We applied Triamcinolone Cream on her foot.

We washed right breast with gauze sponges and Skintegrity.

We applied Cetaphil lotion to face, hands and feet.

Positioned Maureen on her left side.

6:20 pm: Maureen is in bed.
