• 4:00pm Maureen sits comfortably having her fruit snack.

• 4:50pm Diaper it’s light wet.

• 5:00pm Dinner: Chicken parmigiana, 3/4 cup of lemon water. Coughed three times.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper with light trace of clear vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: temp: 97.4, pulse: 81, O2: 97.
