• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Saline and Albuterol Neb treatment.

• 4:50pm Diaper it’s light wet.

• 4:55pm Dinner: Salmon with Brussels sprouts, and 3/4 cup ginger tea. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a Small-formed BM, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: Temp: 97.8, Pulse: 88, O2: 96.
