• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 106/59, P 66.

• 4:45pm Diaper it’s dry.

• 4:50pm Dinner: Turkey with gravy, mashed potato, spinach, green beans and carrots, and 1/4 cup of ginger tea. Maureen yam, yam, yam! Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.
