• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 118/62.

• 4:45pm Diaper it’s dry.

• 4:50pm Pesto chicken with squash spaghetti, sweet potato, and half cup of lemon water. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with very small-clear vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Redness on Maureen’s left arm looks the same as in the morning, applied hydrocortisone cream.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.
