• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 110/65, P76.

• 4:45pm Diaper is light wet.

• 4:55pm Dinner: Sloppy Joe with potato salad, and 4oz ginger water. Coughed once.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a small-formes BM, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

  Pulse: 72

  O2: 99

• Positioned on her left side, Maureen goes to sleep for the night.
