• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 105/65, P78.

• 4:45pm Diaper is light wet.

• 4:55pm Dinner: Salmon with mashed potatoe and carrot, and 4oz of lemon water. Coughed once.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. Applied Aquaphor oilman to her right temple redness spot.

• Vitals:

  Pulse: 73

  O2: 98

• Positioned on her left side, Maureen goes to sleep for the night.
