• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 101/62, P62.

• Maureen had 4oz of lemon water.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Sausages, spinach and cheese casserole, and 4oz of lemon water.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper, applied Cavilon barrier cream, also applied Calazinc to left labia and near anal redness spots. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

  Pulse: 62

  O2: 97

• Positioned on her left side, Maureen goes to sleep for the night.
