• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 103/61, P 69.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Mac and cheese with ham and carrot, and 1/4 cup of ginger tea. Coughed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper with a very small-soft BM, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. Clean/dry redness spot by Maureen’s right-posterior underarm and on by her abdominal fold, applied nystatin powder.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals:

  Temp: 96.8

  Pulse: 68

  O2: 97

• Maureen consumed 28 ounces of fluid.
