• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 101/60, P75.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Lemon chicken, asparagus, carrots, spinach, and half cup of water. Coughed twice, suction pump used.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with small-yellowish vaginal discharge (no charge in odor), applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Positioned on her left side, Maureen goes to sleep for the night.

• Maureen consumed 38 ounces of fluid
