• 4:00pm Maureen is sitting comfortably in the recliner watching tv.

• 4:40pm Checked diaper, it’s dry.

• 4:45pm Blood pressure: 108/66, P 67.

• 4:50pm Dinner: Rice with chicken, cheese, broccoli and 1/3 cup of lemon water. Coughed once.

• 5:50pm Evening meds: 12.5ml Vimpat, 5mg w/apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Cleaned and changed a md wet diaper, applied Cavilon cream. Wiped clean face and hands, applied Cetaphil cream, including lower extremities. Redness by armpit and arm fold seems better, applied nystatin powder.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

• Vitals: Temp: 97.7, Pulse: 58, O2: 97.
