• 4:00pm Maureen is sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 115/79, P67.

• 4:45pm Diaper is dry.

• 4:50pm Dinner: Fajita casserole with lime chips, and 8oz of lemon water. Coughed four times and sneezed twice.

• 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a very small-soft BM and small-clear vaginal discharge, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities. Applied nystatin powder to both fading redness spots, on her abdomen and her right arm pleat.
