I have updated the above referenced document to reflect the change back from 5mg to 10mg of Olanzapine (Zyprexa) taken at night just prior to bed.

Copies of this document will be inserted into the three plastic sleeves in the house—one on the left side of the refrigerator and the other two stored in a FedEx envelope in the cabinet above the refrigerator on the right side.
