Updated Emergency Info & Contact Sheet - v 210503

I have attached the updated version of the Emergency Information and Contact Sheet. This version reflects the complete elimination of Escitalopram (Lexapro) and the reduction in Olanzapine (Zyprexa) from 10MG per day to 5MG per day given in the evening just prior to bed.

We reduced the Olanzapine (Zyprexa) to 5MG beginning on Sunday May 2, 2021. I placed hard copies of this document in the three plastic sleeves in the house—on left side of refrigerator and in the FedEx envelope in the cabinet above the refrigerator.
