Updated Emergency Info & Contact Sheet - v 210416

I have attached the updated version of the Emergency Information and Contact Sheet. This version reflects the change to the dosing of Escitalopram (Lexapro) from 10MG per day to 5MG per day given in the morning.

We made this change beginning on Saturday April 10, 2021. I placed hard copies in the three plastic sleeves in the house—on left side of refrigerator and in the FedEx envelope in the cabinet above the refrigerator.
