• 4:00 pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 100/60, P 80.

• 4:45pm Diaper is dry and clean .

• 4:50pm Dinner: chicken fajita casserole and half cup of water. Coughed twice.

• 5:50pm Administered 12.5ml vimpat and 5mg Olanzapine with apple sauce.

• 6:00pm Oral care.

• 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper with a small-hard BM, pplied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• 6:50pm Positioned on her left side, Maureen goes to sleep for the night.
