  • 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

  • 4:30pm Blood pressure: 104/60, P 70.

  • 4:45pm Diaper is dry.

  • 4:50pm Dinner: Spicy pork tenderloin, potato salad, corn, and 1/3 cup of lemon water.

  • 5:50pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

  • 6:00pm Oral care.

  • 6:20pm Transferred Maureen to the bedroom. Clean/change a heavy wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

  • 6:50pm Positioned on her left side, Maureen goes to sleep for the night.

  • Vitals:

    Temperature: 96.5

    Pulse: 72
