• 4:00pm Maureen’s sitting comfortably in her recliner watching a game show.

• 4:30pm Blood pressure: 110/70, P 69.

• 4:35pm Administer 800mg Acyclovir (crushed) with apple sauce.

• 4:50pm Dinner: Chicken with sun dried tomato, cauliflower and beans, and half cup of electrolyte. Coughed twice.

• 5:30pm Administered 12.5ml Vimpat and 5mg Olanzapine with apple sauce.

• 5:35pm Oral care.

• 5:45pm Transferred Maureen to the bedroom. Clean/change a medium wet diaper, applied Cavilon barrier cream. Wipe/clean hands and face, applied Cetaphil cream, including lower extremities.

• Positioned on her left side, Maureen goes to sleep for the night.
